PC only: the console exists on the Steam/GOG/Epic PC release, not on Xbox 360 or PlayStation 3. Load a save or start a new game.
Press the tilde key (~), just below Esc and left of the 1, to open it. Type a command and press Enter; commands aren't case-sensitive.
To target an NPC or object, open the console and left-click it. Its RefID shows at the top, and commands like kill or unlock now apply to it. Click again to deselect.

Set a quantity, pick an item or perk, copy the exact player.additem / player.addperk command. Covers the most-searched weapons, armor, aid, chems, ammo, caps and perks. A few hundred codes, not every trivial misc item nobody's ever looked up.
Base game items use IDs starting 00. DLC items start xx, which you replace with the add-on's two-digit load order (default: Dead Money 05, Honest Hearts 06, Old World Blues 07, Lonesome Road 08, Gun Runners' Arsenal 09). Anything that needs this is flagged DLC below.
